Virtual Membership Agreement
I am committed to creating an alliance with you that serves you in getting more of what you want. Please read the following agreement, policies and procedures regarding our virtual group coaching engagement. Sign and return denoting your understanding and agreement. Your participation in the Series requires this signed Agreement be returned before your first session.
Monthly Duration
The virtual sessions will take place for 1 hour each month and transmitted via our Private Group on Facebook Live.
Fees/Payment Procedures:
About our financial agreements, the rate for your virtual series is $29.00 per month.
By signing this agreement, you authorize Mosaic Coaching Solutions to bill the card on file each month.
Meeting Time:
Our virtual sessions will take place the second Tuesday of each month 6:30pm – 7:30pm pacific time. Please use a calendar converter to adjust for your time period.
Absence:
If you need to miss a session, please visit our private Facebook page for the recording. There will be no refunds or reschedules for missed appointments.

Clients Responsibility:
It is the client’s responsibility to be an active participant in this series. To ensure your progress, requests will regularly be made of you. It is up to you to put forth your best effort and to engage fully. I ask that you come prepared to each session and bring your questions.
Coach Disclaimer of Liability:
The services to be provided by the certified professional coach, Michele Davenport, are group tele-coaching. Services may include values clarification, brainstorming, examining modes of operation in life and at work, asking clarifying questions and making empowering requests.
DISCLAIMER: Client understands and agrees that Michele Davenport is not an employment agent, a business manager, or financial analyst and that she has not promised or obligated to 1) procure any employment, business or sales for the client; 2) perform any business management functions; or 3) act as a psychotherapist, providing psychological counseling, psychoanalysis or behavioral therapy. Any and all actions resulting from the virtual group sessions are the responsibility of the client.
ICF Permission:
On a regular basis, I will renew or pursue increasing my International Coach Federation credential. By signing this agreement, you also give me permission to include your name, contact information, time period of our engagement and total hours to the ICF as requested. Only this information will be provided. Content of the virtual sessions is not included and held completely confidential.
